The Turning Point of Chester provides individualized case-management services to help people identify their needs, establish personal goals, overcome barriers, and continue moving forward. We understand that every person’s journey is different, so we meet each individual where they are and develop support based on their circumstances.
Case-management services may include:
Individualized goal planning
Recovery support and relapse-prevention resources
Housing assessments and assistance locating suitable housing
Connections to transitional and permanent housing opportunities
Referrals for counseling, mental-health, and substance-use services
Employment preparation and connections to job opportunities
Assistance accessing identification and important documents
Transportation coordination
Connections to healthcare and community resources
Life-skills and financial-management support
Fatherhood and family-reunification assistance
Advocacy, encouragement, and continued follow-up
Case management is not limited to participants in our residential programs. Eligible individuals from the surrounding community may also have the opportunity to receive case-management services, referrals, and connections to appropriate resources.
For residential participants, case management often begins before entry into our 90-Day Residential Program and may continue as they move into supportive housing, i58’s 18-Month Transitional Living Program, apartment living, or other community-based housing.
Our goal is to provide a consistent continuum of care so individuals do not have to face difficult transitions alone. We continue walking alongside them as they work toward recovery, stability, self-sufficiency, stronger family relationships, and a permanent place to call home.
